Perioperative cardiovascular evaluation for non-cardiac surgery

Abstract
Cardiac complications contribute significantly to morbidity and mortality in non-cardiac surgery. The American College of Cardiology (ACC) and the American Heart Association (AHA) have published and revised the ¡°Guidelines on Perioperative Cardiovascular Evaluation and Care for Non-cardiac Surgery¡±. This article will focus on preoperative assessment of cardiac arrhythmia. To evaluate the perioperative cardiac risk, the following information is needed: whether the patient has any active cardiac conditions or comorbid diseases, the risk of the surgery itself and the patient¡¯s functional status. Most asymptomatic ventricular arrhythmias, including couplets and non-sustained ventricular tachycardia, are not associated with an increase in cardiac complications after non-cardiac surgery. However, patients who develop sustained or non-sustained ventricular tachycardia during the perioperative period should be referred to a cardiologist for further evaluations. Beta-blockers can reduce mortality and the incidence of cardiovascular complications including the development of arrhythmias.
